LSF is looking for a talented  House Coordinator who wants to make an impact in the lives of others.

The schedule for this role requires overnight shifts and weekends!

Purpose and Impact:

Plans, coordinates, and ensures that all youth living in their assigned group home are actively working towards the goals set in their service plan. The House Coordinator actively makes all necessary appointments with outside agencies as indicated in the service/treatment plan and follows up with the direct care team to ensure the youth attended the appointments and schedules follow-ups as needed. This individual is expected to take an active role in the development and fostering of therapeutic relationships between youth and staff to provide for effective treatment. The House Coordinator is expected to display and maintain a high standard of ethical behavior and professionalism and function as a member of a therapeutic team in providing a balanced approach to planning and assessment for youth in care.

Essential Functions:

  • Coordinates all resident intakes and discharge paperwork and electronic files including follow through on all pertinent document receiving as well as medication intake counts and discharge counts. 
  • Set all appointments for youth in our program in a timely manner.
  • Organize transportation for appointments with residential services director.
  • Keep up to date medical, dental, and all other appointments logged on master sheet.
  • Responsible for resident blue book/electronic file creation and completion to meet governing standards.
  • Responsible for all resident baby book creation and completion to meet governing standards. (These books are original legal documents and move with the child)
  • Facilitates community surveys distribution and collection for accreditation and analysis the results and reports at staff meetings.
  • Ensure all visitors, volunteers are signed in, screened, and wearing a name tag when visiting anyone on campus. 
  • Check in all donations then make sure they are distributed to proper locations. Daily organization of incoming donations.
  • Welcomes visitors, accepts donations, complete IRS donation form, enter completed data onto donor card at front desk. Those cards should be given to marketing director once a week. 
  • Greets and directs all volunteers on property. Identify if they are supervised or unsupervised and take them to their location. 
  • Email Community Engagement Manager regarding anyone interested in becoming a volunteer, needs a speaker, or would like to do a 3rd party fund raiser for HOME.
  • Ensure compliance with DCF and accreditation guidelines.
  • Assist with licensure and accreditation preparation and record keeping. 
  • Ensures all resident child daycare enrollments.
  • Provides administrative support to directors.
  • Answering phone calls and directs calls to appropriate persons or takes messages and sends emails when needed.
  • Reads and analyzes incoming memos, mail, checks, submissions, and reports to determine their significance and then distributes to the appropriate individuals.
  • Performs general office duties, including inventory and ordering of supplies with monthly spreadsheet when items were ordered and have arrived. Receipts are forwarded to Office Manager.
  • Oversee Christmas budget and shopping with Program Coordinators.
  • Adheres to and promotes all elements of Maternity/Group Home Quality Standards.
  • Maintains companywide calendars for all residents and staff daily. 
  • This position acts as the liaison between the Summerfield & the Belleview office for optimal communication.
  • Provides responsive, high quality customer service to Community Based Care Lead Agencies, the Department of Children and Families, the Department of Juvenile Justice, the Courts, the School Board, and other representatives of outside agencies and members of the public by providing accurate, complete, and up-to-date information, in a courteous, efficient and timely manner.

Physical Requirements:

  • Valid Florida Driver’s License and Insurance.
  • Ability to travel locally, and out of the area, in the execution of professional duties, trainings and/or conferences.
  • Ability to operate a computer, sit for long periods of time, and develop coherent written correspondence and progress notes.
  • Ability to adapt to irregular hours, perform some light lifting, and be flexible to rotate on call, as needed.
  • Certified First Aid/CPR, and crisis intervention.

Education:

High school diploma, associate degree or higher preferred

Experience:

  • 3-5 years of related experience; or equivalent combination of education and experience
  • Also required is a thorough knowledge of residential treatment programs for adolescents, and knowledge of assessment methods and child centered services.

Skills: 

  • Good motivational and social interaction skills to enable staff to participate as an effective team member in a treatment program. Understanding human behavior and the processes required facilitating change. Excellent writing skills including the ability to produce comprehensive narrative reports; must be capable of editing reports written by other staff to produce final report. Must have excellent skills in verbal communication; good organization and time management skills and be capable of operating a personal computer including Microsoft office tools. Ability to function in a team setting, and professionally serve a diverse multicultural client population.
  • Dependability, discretion and good judgment, ability to communicate effectively with clients, parents, co-workers, volunteers, governmental and other agencies and the general public.

Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.

  • Accurate, complete, and timely submission of reports and documents required by LSF.
